Case Management Services

Our case managers provide participants with a holistic approach to their needs, goals and lifestyle choices whilst developing and maintaining rapport, ensuring the participants voice is heard and supported.

Case managers work closely with fund managers whilst also building close relationships with participants family, medical professionals, guardians, and other relevant support services. This interdisciplinary approach to service delivery, promotes a coordinated approach to decision making allowing the participant to make informed choices relating to their life.

Case management has experienced another successful year. Staff have continued to service participants in an effective manner while accommodating the challenges that covid19 has created. Despite these challenges we have experienced growth within the program enabling us to employ an additional part time staff member.

Referrals have been received for 14 new participants along with repeat referrals for existing participants who have faced a new set of challenges. Common themes for referrals include housing and financial assistance and navigation of service pathways ensuring clients needs are met by the most appropriate agency.

Our case managers pride themselves on the relationships they develop and foster with our participants, external service providers and referral sources to ensure best possible outcomes. Our vision for the program is continued growth which includes the exploration of new partnership opportunities.
