What is our Case Management service?
Our Case Management service provides personalised support to help people living with Acquired Brain Injury (ABI) build life skills and better access the community.
Qualified case managers work together with individuals, their families and service providers to achieve their goals and areas of concern.
Services Available
We can help you with:
- Finding services, therapies and activities to help achieve goals
- Managing finances, housing applications and maintaining tenancies
- Organising referrals and services
- Employment services including Centrelink liaison
- Enrolling in educational programs and accessing student support services
- Attending medical and specialist appointments
- Connecting with social and support groups
- Community integration
Our case managers work closely with service providers for you, making sure they are delivering the required services.
Who can use the Case Management service?
People of all ages living in South Australia with an ABI. This is on a fee for services basis.
